Q108 ‘My Tattoo Must Go’ Contest Rules
Beginning at 6am on March 11, 2014 through March 30, 2014, you can attempt to win a laser tattoo removal service. The contest is open to those who hold a valid article of state identification and are 18 years of age or older and those 18 or older with a valid Military I.D.
To attempt to win, you must register online at Q108 dot com by submitting a picture of the tattoo to be removed. The picture must be fitting of the contest theme, and the tattoo must be on the person entering. Q108 reserves the right to refuse any submission.
Listeners will be encouraged to vote for their favorite picture at Q108 dot com. There will be a voting period from 12:00pm on March 24th, 2014 through 11:59pm on March 30th, 2014. Listeners can vote once per day during the voting round. Voting is limited to one vote per verified email address. If an entrant receives irregular votes from the same user or users, including but not limited to, votes generated by a robotic, programmed, script, macro, other automated means or other source, Q108 reserves the right to disqualify the entrant in its sole discretion. To participate in voting you must become a registered user through uPickem.
The entrant with the highest number of votes will win the grand prize gift certificate offering a choice of nine services from Cumberland Laser Center valued up to $4500. The winner may choose one (1) of the following Treatment Packages:
1. Laser Tattoo Removal (up to 36 square inches) package of up to 12 sessions
2. Laser Hair Removal package of 6 sessions
3. Laser Facial package of 6 sessions
4. VIP Laser Facial package of 6 sessions
5. Intense Pulsed Light Photofacial package of 4 sessions
6. Intense Pulsed Light Treatment of Facial Veins package of 4 treatments
7. Intense Pulsed Light Treatment of Décolleté package of 4 sessions
8. Microdermabrasion package of 6 sessions
9. Dermaplaning package of 6 sessions
All treatments are subject to review of health history and medications. Services cannot be offered if not appropriate, based on health status. Treatments must be started within thirty (30) days of winning certificate. All treatments (except for tattoo removal) must be completed within one (1) year of the first session. The One Year Laser Hair Removal guarantee is not extended with the free package. Certificate cannot be extended to other services offered in the center, retail products, prescription products, or services provided by Dr. Lee. Certificate cannot be used for past purchases. No cash value. Non-transferable.
The grand prize and the winner will be announced on March 31st, 2014. Prizes must be claimed no later 30 days after the end of the contest. Complete Q108 contest rules apply and are available at Q108 dot com.
No purchase necessary. One valid entry per contestant.
This contest is void where prohibited by law.
Winners will be notified by email. If a winner is unreachable after seven (7) days, or if that winner is unavailable for prize fulfillment, an alternate winner will be selected. If Q108 cannot find an eligible winner for the prize, that prize will not be awarded.
Odds of winning depend on the number of eligible entries received.
Automated voting of any kind is not allowed. Detection of said automated voting will lead to votes being voided.
Only one registered account per voter. If multiple accounts are detected for a single voter, they will be disabled and votes disqualified.
Q108 reserves the right to substitute any prize with another prize of equal or greater value.
Q108 reserves the right to restrict the local winners to only persons whose primary residence is within a 60 mile radius of the affiliate's' office.
Employees of Q108, its respective parent, affiliates, subsidiaries, advertising sponsors and promotional agencies, and the immediate family members of each are not eligible.
Contest entrants agree to abide by the terms of these Official Rules and by the decisions of the contest commissioners, which are final on all matters pertaining to the contest.
Entrants further grant to Q108 the right to use and publish their proper name and state online and in print, or any other media, in connection with the Contest.
Q108 reserves the right to use any and all information related to the Contest, including information on contestants obtained through the contest, for marketing purposes or any other purpose, unless prohibited by law.
The Contest and all of the related pages, contents and code are copyright, UPICKEM, and/or Q108. Copying or unauthorized use of any copyrighted materials, trademarks or any other intellectual property without the express written consent of its owner is strictly prohibited.
Each winner, by acceptance of the prize, agrees to release all Sponsors, and their parent and subsidiary companies, their officers, directors, employees, agents, shareholders, affiliates, suppliers, distributors, and advertising agencies from all liability, claims, or actions of any kind whatsoever for injuries, damages, or losses to persons and property which may be sustained in connection with the receipt, ownership, or use of the prize.
UPICKEM and Q108 are not responsible for Internet crashes or slowdowns caused by network congestion, viruses, sabotage, satellite failures, phone line failures, electrical outages, natural disasters or acts of man or God.
All results posted are unofficial until winners respond to e-mail notification.
Each winner is responsible for all federal, state and local taxes and will be required to complete an IRS form W-9. Sponsor reserves the right to withhold prizes until completed form W-9 is received.
This contest is not intended for gambling. If it is determined that a contestant is using the contest for gambling purposes they will be disqualified and reported to authorities.
Q108 reserves the right to change the rules at any time.
